The rates below indicate the percentage of Patients who have required an unplanned readmission within 30 days of discharge. Fewer readmissions can show that a hospital is doing its best to prevent complications, provide clear discharge instructions to Patients, and ensure Patients make a smooth transition to their home or another setting such as a nursing home. Our goal is to make sure our Patients go home at the right time with the right resources.
Appropriate Care Score
The Appropriate Care score summarizes whether Patients received all of the recommended treatments based on their specific conditions. Since each Patient is unique and may not be eligible for every type of care for a condition, this score accounts for individuality by examining each Patient's care one episode at a time.
Survival rates represent the percentage of Patients who enter the hospital for a particular condition who leave the hospital alive. This is often dependent on how ill the Patient is at the time they are admitted. Our goal is to have a survival rate the same or better than what is expected based on the severity of the Patient's illness.
