How to prepare
Advance Care Planning assures you that your choices are heard and followed in life-threatening situations when you are unable to speak for yourself. It involves:
- Completing an Advance Directive form, a legally recognized document detailing your choices.
- Naming a Patient Advocate to act on your behalf to carry out your health care choices if you cannot voice them.
- Discussing your choices with your family and health care provider.
- Putting your Advance Directive in a safe, easy-to-find place.
