Application Process for CME
- Obtain a blank copy of the Application for Designation of Category 1 Credit from the CME Office by calling either 517.364.2197 or 517.364.2878.
- Read and complete the application in its entirety. If your application is for a educational event such as
Grand Rounds or an ongoing series-type, be sure to include with your application a minimum of six weeks
worth of topics, speakers and a list of their objectives and attach the following supporting documentation:
- Signed Conflict of Interest and Disclosure Statements from each speaker
(regardless if the speaker is a resident, from out of town, or is on the SHS medical staff)
- Copies of your speaker's Curriculum Vitae or Biographical Sketch
- List of Objectives for the event from your speaker
- One-time events also require this information and a copy of the proposed brochure. Do not send out
an announcement or a brochure stating that you have applied for CME Credits, or that CME Credits are pending, you must wait until you have
approval. Failure to adhere to this rule will result in a denial of CME Credits and support for your activity.

- When Honorariums, Commercial Support, Display Fees, or Beverage/Food/Miscellaneous Expenses is part of your CME activity, an additional set of documentation is needed such as a Budget, W-9 Request for Taxpayer ID/Certification, and a Letter of Agreement for Commercial Support. In these cases, either the Facilitator or Commercial Representative will need to meet with the CME Staff to complete the forms.
{Note: One-time events also require this information but obviously just for the one event.}
- Submit the entire application (a minimum of) six (6) weeks in advance of the actual educational event to the CME Office with required supporting documentation so that it can be reviewed by the CME-Library Committee for approval or disapproval.
{Note: Applications will not be approved for CME sponsorship if it is missing supporting documentation.}
- Once the CME Office receives your application they will review it for completeness and prepare it for the CME Committee's review. If your application is missing any supporting documentation our office staff will notify you immediately.
- The CME/Library Committee meets the second Tuesday of every month to review applications.
