A program inspired by personal stories from local community members, the Navigation Hub is a one-stop-shop for helping patients who have a UM Health-Sparrow primary care provider access community resources, obtaining educational materials, and navigating the health system.
We can assist you in accessing resources such as:
- Housing assistance
- Transportation aid
- Public benefits assistance
- Meal programs
- Senior services
- Other needs as identified
For information on the services provided and how our team can assist, reach out to our team of Resource Coordinators today. Our Resource Coordinators are UM Health-Sparrow team members dedicated to assisting patients with general questions, accessing community resources, and navigating the health system. This service is available to UM Health-Sparrow Primary Care patients with no out of pocket cost.

Whether for yourself or a loved one, the Navigation Hub can provide resources and access to individual services to best meet your needs, eliminating the confusing maze of information, and simplifying the process for you. At this time, we are only providing these services to patients who have a UM Health-Sparrow Primary Care provider.
The team can provide information on community resources that can be helpful in reducing barriers to care. Examples of resources offered by the Navigation Hub include: housing assistance, transportation aid, public benefits assistance, meal programs, senior services, and other needs as identified.
Resource Coordinator services are available at no out-of-pocket cost to the patient; some insurances do cover this service as well.
