A program inspired by personal stories from local community members, the Navigation Hub is a one-stop-shop for helping patients who have a UM Health-Sparrow primary care provider access community resources, obtaining educational materials, and navigating the health system.

We can assist you in accessing resources such as:

  • Transportation aid
  • Public benefits assistance
  • Meal programs
  • Senior services
  • Other needs as identified

Our Resource Coordinators are UM Health-Sparrow team members dedicated to assisting patients with general questions, accessing community resources, and navigating the health system. This service is available to UM Health-Sparrow Primary Care patients with no out of pocket cost.

At this time, we are only providing these services to patients who have a UM Health-Sparrow Primary Care provider.

What services are offered by the Navigation Hub?

The team can provide information on community resources that can be helpful in reducing barriers to care. Examples of resources offered by the Navigation Hub include: housing assistance, transportation aid, public benefits assistance, meal programs, senior services, and other needs as identified.

How much are the services provided by the Navigation Hub?

Resource Coordinator services are available at no out-of-pocket cost to the patient; some insurances do cover this service as well.
