To schedule a counseling session, please call 517.364.2387. Your counseling session will
last approximately one hour. Feel free to invite a family member or significant other to join
you in the session at no additional cost.
In preparation for your appointment, please fill out the Nutrition Questionnaire and keep a three-day food diary.
This can be FAXED to 517.364.3191 or you can bring it with you to your appointment. The diary should consist of writing down everything you eat
and drink along with portion sizes.
For example:
1 slice toast
1 teaspoon margarine
1/2 cup orange juice
1 cup black coffee
The Nutrition Counseling Office is located in the lower level of the main hospital, across the
hall from the cafeteria. Sparrow campus visitors and patients should park in the Parking Deck
located on the corner of Michigan Ave. and Pennsylvania Ave. Parking fees are $2.00 and are collected
as you exit the deck. Click here for more parking information, including valet parking.
We will attempt to bill your insurance carrier, but you may wish to check with them to verify
coverage of this service. If not covered, the fee of $105.00 may be paid at the time of your
appointment by cash or check.
