How to Sign Up: Make your Sponsorship commitment: (Click here to review Sponsorship Packages) Fill out and submit the Online Sponsorship Form OR Download the Sponsorship Form, complete and mail or fax to: Sparrow Foundation 1110 W. Michigan Ave. Lansing, MI 48912 The Sparrow Foundation office will then invoice your company for the level of commitment you decide upon. Register Golfers - Based on your sponsorship level, register one or two teams of golfers using the online form.
