STEP 1: Sponsorship Sign Up

Deadline: August 18, 2008

You have three options to send in your sponsorship registration:

1. Online Registration (Credit card payment)
2. Online Registration (Please send invoice)
3. Download the registration form (PDF)
   Complete the form and mail to the Sparrow Foundation office with your check or credit card payment.
   Adobe Reader required to view this form. Click here if you need to download Adobe Reader.

STEP 2: Email your company's logo

Deadline: August 18, 2008

Email your logo to lisa.weber@sparrow.org

Please send your logo in any of the following formats: JPG, GIF, TIF, EPS. The logos will be used for printing on signs and display on the web site.

STEP 3: Register the names of your foursome

Deadline: August 18, 2008

Use the two options below:

2. Download the form
   After you print and complete the form, mail it to the Sparrow Foundation office.
