Home Health Services
Billing/Financial Concerns:
Primary emphasis at Sparrow Health System is to provide you with quality, compassionate and cost effective health care.
If you have an insurance coverage question or a concern about a financial issue regarding your hospital stay, please contact one of the patient financial counselors.
Sparrow Health System requests that you make arrangements for payment of deductible and co-pay amounts prior to discharge.
The financial counselors are there to assist in determining these amounts, and they can work with you to arrange for payment.

These counselors are available Monday - Friday, 8 a.m. - 4:30 p.m. and are located on the main floor in the Admitting Department. Please call 517.364.2737
for assistance. Our financial counselors may assist you in determining if you qualify for financial assistance and will provide guidance in filing the proper paperwork. Sparrow Health System will bill insurance companies whenever that information is provided and there are benefits available. Billing or financial concerns after discharge may be discussed with the associates at Patient Financial Services at 1.800.968.2619.
Discharge:
When you are officially discharged a hospital volunteer or staff person may take you out of the hospital in a wheelchair to assure your comfort and safety on the way to the car. A family member or caretaker can help you from there. Before leaving the hospital, make sure you understand your needs for home care and ask questions regarding follow-up visits. Also make sure you thoroughly understand your physician(s) and/or health team's instructions and recommendations.
Discharge Planner/Social Worker:
Sparrow's patient Support Services Department provides a wide range of services. These services are designed to help and support all other health care activities. The staff includes experienced social workers and nurses who become involved early in the hospital stay to encourage the patient and family to participate in the treatment and discharge plan. These services include:
- Counseling to help with lifestyle changes which may be necessary because of the patient's medical condition and helping patients and families affected by chronic and terminal illnesses.
- Information and Referral assistance to connect patients and families with community resources related to insurance, legal and financial programs.
- Discharge Planning which includes help in obtaining medical equipment such as hospital beds, wheelchairs, commodes, canes, walkers, feeding pumps, and glucose monitoring machines.
- Skilled Home Care Services which include IV therapy, nutrition and ventilator management.
- Home Care Support such as Hospice, home companions, housekeeping and other chore services.
- Placement Services to nursing homes, foster care, and transfers to other hospitals.
- Special Transportation Arrangements for wheelchair and stretcher transports by ambucab or ambulance.
The professional staff considers the social values and economic environment in which the patient lives and works. The patient and family are involved in decisions regarding the needs identified, the plans for intervention and the providers to be utilized for services and/or referrals. This information is shared with the health care team and contributes to the treatment and rehabilitation of the whole person. Education and Support:
Many patient education and support services are offered. Nurse clinicians are available for diabetes, cancer,
ostomy, and cardiac education and support. There are also these support groups available for special needs:

Home Health Services:
Several home services are available to help you with special health requirements. Usually physicians make the referrals for services that may be needed by the patient at the time of discharge. The Patient Support/Discharge Planning staff will then confer with you or your family members and explain the services. You may select the agency of your choice to provide the home health services. Sparrow Regional Medical Supply
Provides supplies for most health care needs. This service may also arrange for non-emergent transport by ambulance and for transportation for shopping or personal needs. Open Monday - Friday, 8 a.m. - 6 p.m., and Saturday, 9 a.m. - 3 p.m. Call 517.371.2115 or 1.888.7WE.HEAL.
Sparrow Home Care Network
Certified Services provides skilled home health care as prescribed by your physician. Open Monday - Friday,
8 a.m. - 5 p.m. (Also available evenings, weekends, and holidays). Call 517.364.8600.
