What is Cardiac Rehabilitation?

Cardiac Rehabilitation (rehab) is an individualized program of exercise and education to help patients with heart disease obtain an optimal state of health. The Cardiac Rehabilitation staff works with each patient and their physician to develop an individualized plan of care.

Cardiac Rehab consists of two parts:

  • Exercise Training to help you exercise safely, strengthen your muscles and get your energy back.
  • Education and Counseling to help you understand your heart condition and find ways to change unhealthy habits to new, healthy ones.

Who Can Participate in Cardiac Rehabilitation?

In general, patients are eligible to participate if they have or have had:

  • Bypass surgery
  • An angioplasty or stent placement
  • A heart attack
  • A heart transplant
  • An established diagnosis of coronary artery (heart) disease
  • Stable angina

A Physician Referral is Required.
Physicians can make a referral by calling 517.364.5364.
The program is covered by most insurances.

The Three Phases of Cardiac Rehabilitation

Phase 1

Recovery from a heart event is a lifetime process that begins with the care given immediately after the heart event. Once the heart is out of immediate danger, the patient's role in recovery begins. Phase 1 Cardiac Rehabilitation takes place while the patient is in the hospital and includes: Education about heart disease, procedures, risk factors, activity level and the recovery process. Assistance with increasing the patient's activity level during the hospital stay.

Phase 2

Cardiac Rehab occurs on an outpatient basis after the patient has been discharged from the hospital. Phase 2 Includes: Exercise: Individually prescribed and monitored exercise sessions help patients return to daily activities or work and improve quality of life Education: Weekly classes are held covering many topics important for heart healthy living. Encouragement & Support for the patient and their family.

Phase 3

Patients are provided with a plan for activity and lifestyle changes to continue their heart healthy living day to day. In this phase, patients are independent in their exercise sessions, but have the Cardiac Rehab staff available at all times.
