Our Home Hospice teams partner with patients and their families to provide loving, high-quality care. Our three teams serve Clinton, Eaton, Ingham, Ionia, Gratiot and Montcalm counties, plus parts of Jackson and Shiawassee counties.
The Home Hospice Teams, located in Lansing, St. Johns and Carson City, offer a comprehensive array of services, including:
- Medical care
- Nursing care
- Support, counseling and community services
- Physical, occupational and speech therapy
- Inpatient respite care – temporary relief to families caring for loved ones
- Inpatient care for symptom management at Carson City Hospital, Sparrow Clinton Hospital and the Hospice House of Mid-Michigan
Our hospice team provides complete support for the family and can assist and train others to provide care. In addition, volunteers provide companionship for patients and respite for families, and assist with transportation, run errands, offer gentle massages, listen to and record life stories and meet other individualized needs of both the patient and family members. Our staff and volunteers live in the communities that we serve.
The patient's physician directs pain management and symptom relief medications and provides overall medical supervision. A registered nurse serves as a case manager and is responsible for making assessments vital to the patient's care, intervening with the patient’s physician to update him/her on the patient’s condition, and order medications and/or other treatments. The registered nurse also teaches the care of the patient and coordinates the care of the other team members. A social worker provides support and counseling and is a resource for community services. If requested, clergy provide spiritual counseling and support.
