What is diabetes?
Diabetes, also called diabetes mellitus, is a disease in which the amount of glucose (sugar) in the blood is too high. Insulin is vital for converting sugars into fuel so your body
can function properly. When these sugars are not turned into energy, you body responds by creating more glucose from stored fats. Over time, these sugars accumulate in the bloodstream. If the condition is left untreated, it
can cause very serious complications, including heart and vascular disease, kidney, eye and nerve damage. While many people can control their diabetes by strictly following their physician's directions, others
have a more difficult time.

Home Care nurses are specially trained to treat the diabetic. The Sparrow Home Care Team will assess all facets of your daily activity and dietary
intake. They continue to review your physician's instructions and dietary needs. A dietician is part of the care team assisting in review of diet logs.
Many times, specialty shoes may be needed to assist in ambulation and reduce pressure sores. The Sparrow Home Care Team can assist in this process as well.

Treatment Options for Diabetes
The key is to keep your sugar levels within your target range by maintaining a healthy lifestyle and following your physician's directions. Diabetes will either be treated
through insulin injections, oral medication or diet. The Sparrow Home Care Team assists with instruction and reinforces education of checking your blood sugar, administering an injection or monitoring
and educating you on medication regime.
For those with diabetes, prevention is a must. The Sparrow Home Care Team takes every measure to assist you in checking elements of our lifestyle that affect your health,
which includes stopping smoking, exercising regularly, eating a balanced diet (low in carbohydrates, fat and sugars), and maintaining a healthy weight, cholesterol level
and blood pressure level. While it is important that all people lead a healthy lifestyle, it is especially important if you have diabetes. All of the previously mentioned risk factors compound the severity of diabetes.
Payment for Services
- Medicare Part A traditional covers at 100% with no co-pays or deductibles when regulatory criteria is met
- Medicaid covers 100% when the home is the most appropriate place to provide care
- HMO policies dictate coverage criteria, co-pays, and deductibles
- Blue Cross/Blue Shield and other private
insurance policies dictate co-pays, deductibles and requirements for
services

- All ages are accepted by Sparrow Home Care
- Patients have the right to choose their home care provider
- Initial assessments provided to ensure you know up front your financial obligations or coverage criteria
- Referrals are accepted 24 hours a day, 7 days a week, 365 days a year
- Referrals are accepted when being discharged
from any hospital, rehabilitation center, nursing home or directly from
your physician office
