Proper labeling of patient specimens is a critical component of patient safety and quality laboratory results. A minimum of two patient identifiers (first and last name and either patient history number or date of birth) must be on the sample before submission to the laboratory. Samples received in the laboratory with less than this minimum will be returned to the submitter.
