Inpatient Rehabilitation Admissions Process (Physician Referral)

Referral from a Sparrow Inpatient Unit
Referral from Another Healthcare Facility
Referral from Outpatient Rehabilitation

Admission Process for Referral from Another Sparrow Inpatient Unit:

1. Physician writes referral order in chart.
2. Rehabilitation Nurse Coordinator visits the patient for pre-admission evaluation including:
• information about the patients' past medical and surgical history
• current medical status
• functional limitations
• insurance
• support system and discharge plan
3. The Rehabilitation Nurse Coordinator or the Rehabilitation Physician reviews the information.
4. Information may also be obtained from other members of the Rehabilitation Team or other professionals familiar with the patient's case.
5. Admission decision is based upon the Rehabilitation Program's Admission Guidelines and the patient's insurance requirements.
6. The Rehabilitation Coordinator or the Rehabilitation Physician notifies the patient/physician of the admission decision or makes suggestions for alternate treatment plan.

Admission Process for Referral from Another Healthcare Facility:

1. Physician writes referral order in chart to Sparrow Inpatient Rehabilitation.
2. Facility contacts Rehabilitation Nurse Coordinator at 517.364.2747 and faxes information including:
   
   • information about the patients' past medical and surgical history
   • current medical status
   • functional limitations
   • insurance
   • support system and discharge plan
3. The Rehabilitation Nurse Coordinator or the Rehabilitation Physician reviews the information.
4. Admission decision is based upon the Rehabilitation Program's Admission Guidelines and the patient's insurance requirements.
5. The Rehabilitation Coordinator or the Rehabilitation Physician notifies the patient/physician of the admission decision or makes suggestions for alternate treatment plan.

Admission Process for Referral from Outpatient Rehabilitation:

1. Primary Care Physician calls the Rehabilitation Physician office for an appointment at 517.364.5260.
2. Rehabilitation Physician gathers information including:
   
   • information about the patients' past medical and surgical history
   • current medical status
   • functional limitations
   • insurance
   • support system
3. Rehabilitation Physician reviews the information.
4. Admission decision is based upon the Rehabilitation Program's Admission Guidelines and the patients' insurance requirements.
5. Rehabilitation Physician notifies the patient/physician of the admission decision or makes suggestions for alternate treatment plan.
