Sudden illness or disability may mean dramatic changes for you and your family. Adjustment to these
changes can affect your recovery. A medical social worker is available to help address personal questions
or concerns during your stay on the Rehabilitation Unit and to assess or plan for care after your
hospitalization.
Support is also provided to help you cope with chronic illness, disability, or family
crisis. Assistance is available for insurance problems, financial concerns, disability eligibility,
employment or vocational questions, transportation, housing, legal concerns, and patient rights.
The social worker coordinates follow-up services in the home. Skilled nursing care (see
Sparrow Home Care),
medical equipment/supplies, personal care, chores/house-keeping services, and home delivered meals are
support services that can ease your transition to home. If home care is not possible, then discharge
assistance to adult foster care or nursing home placement is provided.
