Case Management & Discharge Planning

Case Management at Sparrow Specialty Hospital is performed by specially trained RN's, Social Workers and Licensed Counselors.

Sparrow Specialty Hospital's Case Managers involvement begins before admittance to our LTACH and extended throughout all patient stays.

The Case Manager is the principal liaison between the patient and their treatment team when formulating a plan of care designed to ensure the patient's needs are being met, including explaining components of that care plan to the patient and their family.

Case Managers work with patients to ensure a smooth and appropriate transition from Sparrow Specialty Hospital to the next step in continuing their healing process.

Intake
As an LTACH, strict guidelines must be followed as to which patients can be admitted to our facility to continue their healing process.

Sparrow Specialty Hospital's Intake Case Managers primary responsibility is to review patients medical history with caregivers and verify eligibility for admittance and treatment here at SSH. Clinically, patients would require vent weaning, IV antibiotic therapy, complex wound care, respiratory or medical needs or cardio vascular/peripheral vascular needs.

Our patients come primarily from short term acute care hospitals in Michigan, but have come from as far away as Florida. We have also received admissions from Doctor's offices, patient homes, skilled care facilities and nursing homes.

Discharge Planning
In order to ensure that patients have a smooth transition back into the community, Discharge Planning begins on Day 1.

While we are a Long Term Acute Care Hospital; protocols, which we are required to follow, still limit our patients stay.

Patients come to us from many different venues and are discharged back to many different venues to continue their healing process. The discharge could be:

  • Home with referrals for:
    • Skilled nursing services
    • Home rehab services
    • Wound care clinic
    • Intravenous infusion clinic
    • Home hospice care
  • To a skilled nursing facility
  • To a sub-acute rehab facility
  • To a short term acute hospital for rehabilitation
  • To a ventilator dependent unit for continued care
  • To an inpatient hospice facility

We will work with patients and families to obtain the best possible outcomes.
