each patient, using a multidisciplinary approach to patient care. The program focuses on patients with impaired
tissue integrity secondary to various factors.

As directed by the attending physician, our healthcare team works collaboratively to evaluate and manage:
co-morbidities, nutrition and hydration, prevention and control of infection, wound healing, functional
maintenance and improvement, pain control, healthy tissue, and psychosocial adjustment. Careful assessment,
monitoring and communication at team conferences are utilized to ensure the patient's timely discharge home or
to a post acute setting.
The medical staff-approved wound care clinical pathway is modeled upon the three phases of tissue repair:
inflammation, proliferation, and maturation. This physiology-based model allows clinicians to identify and
monitor outcomes based upon observable and measurable clinical parameters.
There are four components integral to the pathway: protocols, policies, procedures, and patient education tools. The care and management of patients with slow healing
and sometimes infected wounds is a complex process. Patients admitted to Sparrow Specialty Hospital's Complex Wound Management Program face serious risk of infection and may also
have multiple other medical complications.
Our goal is to facilitate wound healing sufficient for discharge home or to a post acute setting.
