What happens during a patient’s hospital stay is only part of the story. As mid-Michigan’s leading health care provider, we feel it is important that Sparrow takes a leadership role in addressing post-discharge care.
That’s because one of the best predictors of a patient’s long-term post-hospital success is followup care and treatment.
More than 6,000 people come to Sparrow annually due to chest pain and each are potential AMI patients. Sparrow provides expert heart attack care in the areas of medical treatment (clot busting drugs, aspirin, beta blockers, statins and angiotensin converting-enzyme inhibitors) and invasive treatment (angioplasty and stents).
However, simply surviving a heart attack is just the beginning of a long process of recovery.
After discharge, questions about medication, activities, work, cardiac rehab and follow-up appointments arise. A post-discharge follow-up visit with a registered nurse can answer these questions and assist in making sure the patient understands the importance of their medications, discharge instructions and follow-up care.
The Sparrow program provides two home visits and two follow-up calls by a registered nurse during the first 30 days after discharge. The goal is to provide each patient a visit by a specially trained registered nurse 1-2 days after discharge and 4-5 days post-discharge. Another goal: Follow-up calls 7 and 21 days after the patient has left Sparrow. The visits and phone calls are free to patients.
The home visits will consist of the following:
- Physical assessment of the patient
- Reviewing what medications they are taking
- Home safety assessment
- Identifying a care partner with whom the patient can discuss care decisions
- Instruction on signs and symptoms of when to call their physician
- Identification of the patient’s primary care physician and cardiologist
- Ensuring the patient
