Enhancing Quality of Life for Seniors and Their Families

A program inspired by personal stories from local community members. The Senior Navigation Hub is meant to be a one-stop-shop to learn about clinical services, programs, obtain educational materials and access community resources. Based on best practice research, staffing for the Senior Navigation Hub includes a Resource Coordinator and a Geriatric Patient Navigator. The Senior Navigation program is designed to make it easier for seniors, their loved ones, and caregivers to access essential information and services to ensure the best quality of life.

Located within Sparrow Medical Group (SMG) Senior Health, services may be offered at SMG Senior Health, by phone, within the community or by home visit. If you are a Sparrow Medical Group patient interested in services, please contact your Sparrow Medical Provider directly. For all other community members, please contact the Navigation Hub by clicking the email button below or by phone 517.364.8648. The Senior Navigation Hub is available to provide services Monday-Friday from 8:00 AM-4:00 PM.

Navigation Hub Resource Coordinator

The Navigation Hub Resource Coordinator is available at no charge to the senior community. As the Navigation Hub Resource Coordinator, Heather provides clients and caregivers information on community resources, assists in navigating services and builds a communication bridge to reduce any barriers. Examples of resources offered by the Navigation Hub Resource Coordinator include: transportation and housing assistance, food pantry referrals, connections to senior centers/adult day programs and a variety of other community connections.

Geriatric Patient Navigator

The Geriatric Patient Navigator is a Registered Nurse dedicated to serving a small panel of clients as a “surrogate family member". This service includes a free initial home visit to assess if the program is appropriate. Continued services are then offered at a private pay rate of $100/ hour.  Kimberly can provide care assistance ranging from attending appointments, providing wellness checks, assisting with paperwork and other care-coordination activities, all which provide you and your loved one with peace of mind.

What services are offered by the Senior Navigation Hub?

The Navigation Hub Resource Coordinator provides clients and caregivers information on community resources, assists in navigating services & builds a communication bridge to reduce any barriers. Examples of resources offered by the Navigation Hub Resource Coordinator include: transportation & housing assistance, food insecurity, senior centers, smoking cessation and a variety of other community connections.

Geriatric Patient Navigator Services Include:

  • • 1 free initial home visit to determine if the program is appropriate
  • • Reviewing current medications & creation of a medication list
  • • Attending medical appointments and/or procedures
  • • Assistance with and coordination of appointments
  • • With permission of client, keeping family members informed of health status & evolving needs
  • • Taking and keeping track of vitals when requested
  • • Assisting with health-related bills & paperwork
  • • Coordinating prescriptions & durable medical equipment
  • • Coordination with the client’s multidisciplinary care team
  • • Connecting clients with our Navigation Hub Resource Coordinator
  • • Offering a monthly status report to include: current medication list, list of medical providers, services that have been offered that month & plan for next visit
How much are the services provided by the Senior Navigation Hub?

Resource Coordinator services are available free of charge.
Geriatric Patient Navigator services are available at a private-pay rate of $100 per hour.

How can I access Services from the Senior Navigation Hub?

Reach out to your Sparrow Primary Care Provider who can then send a referral for resources to the Navigation Hub.

Can patients who do not have a Sparrow Provider receive services from the Senior Navigation Hub?
