Sixty-four Sparrow Care Network (SCN) physicians and medical offices have been selected as a Patient-Centered Medical Home designated practice by Blue Cross® of Michigan for 2020.
The designees comprise Sparrow Medical Group, Memorial Healthcare practices and other independent physicians who are members of SCN, a physician-led clinically integrated network of over 600 physicians in Mid-Michigan. SCN emphasizes quality and efficient care and ensures patients receive the best possible care at the lowest possible cost.
