Nine practices designated Patient-Centered Medical Homes

Nine Sparrow Medical Group practices have achieved and/or maintained Patient-Centered Medical Home (PCMH) designation through Blue Cross Blue Shield of Michigan.

This designation means that our Caregivers, led by primary care Physicians, are proactively managing our Patients’ care across health care settings – focusing on wellness, disease management and Patients’ unique personal health goals.

  • SMG Eastside
  • SMG Holt
  • SMG Lansing
  • SMG Mason
  • SMG North
  • SMG Okemos
  • SMG Potterville
  • SMG West

To achieve this designation, practices must implement PCMH capabilities that include providing self-management education and support to Patients with chronic conditions, working with Patients to set personal health goals, communicating test results in a timely manner, and providing secure, online electronic communication tools. 

These efforts help transform care for our Patients, improving their well-being and reducing Emergency Department visits and hospital stays.

In addition to SMG practices, 57 Sparrow Physicians Health Network practices have also achieved PCMH designation in 2014.
