Sparrow Medical Group St. Johns (SMG St. Johns) has become the first practice in the United States associated with a critical access hospital to earn The Joint Commission’s Gold Seal of Approval® for Primary Care Medical Home (PCMH) Certification.
The PCMH Certification represents how effectively primary care clinicians and an interdisciplinary team of caregivers work in partnership with Patients to provide comprehensive, coordinated, Patient-centered care.

The Gold Seal of Approval® is a symbol of quality that reflects an organization’s commitment to meeting certain performance standards and to providing safe and effective Patient care. The certification is a special add-on option for Joint Commission-accredited hospitals and critical access hospitals.
SMG St. Johns underwent a rigorous on-site Joint Commission survey in June.
Established in 2013, The Joint Commission’s PCMH Certification is awarded for a three-year period. Continuous compliance with certification standards encourages accredited hospitals and critical access hospitals to ensure Patients receive timely and appropriate treatment. PCMH Certification can potentially result in increased Patient satisfaction, improved Patient outcomes and reduced health care costs.
