Three SMG practices earn statewide honors for diabetes care

Sparrow Medical Group (SMG) DeWitt, SMG Holt, and SMG St. Johns received awards for having among the top diabetes care in the state at the recent Michigan Primary Care Transformation (MiPCT) Project Summit.

Sparrow Medical Group St. Johns earned both Best Overall Diabetes Care and Most Improved Diabetes Care. Sparrow Medical Group DeWitt earned Best Overall Diabetes Care. Sparrow Medical Group Holt earned Most Improved Diabetes Care.

Best Overall awards are given to those ranking in the top five practices in this category in the state. Most Improved Diabetes awards are given to those who have most improved their diabetic measures during the past calendar year in the areas of retinal eye exams, and screenings for risk factors for developing diabetes-related complications.

The Michigan Primary Care Transformation Project (MiPCT) is a demonstration project testing the value of the Patient Centered Medical Home (PCMH) model, which expands access to primary care while improving care coordination. Patient-Centered Medical Home designation means that our Caregivers, led by primary care Physicians, are proactively managing our Patients’ care across health care settings – focusing on wellness, disease management and Patients’ unique personal health goals.

These three SMG practices are among 21 Sparrow-owned practices to achieve the PCMH designation in 2015. They are also among 14 Sparrow Physicians Health Network and eight SMG practices which are participating in the MiPCT demonstration project, the largest PCMH project in the nation. MiPCT recognition reaffirms Sparrow’s ongoing commitment to transforming care, pursuing best practices, and delivering nationally recognized care to improve the health of the mid-Michigan region.
