Two SMG practices recognized for quality care

Sparrow Medical Group Holt and Sparrow Medical Group Mason received awards for their exemplary work to provide quality care at the recent Michigan Primary Care Transformation project (MiPCT) summit.

Sparrow Medical Group Holt was honored as the most improved in the state for overall diabetic quality and Sparrow Medical Group Mason was named one of the top five practices in the state for overall adult and family medicine clinical quality performance.

MiPCT is a project aimed at improving health in the state, making care more affordable, and strengthening the Patient-care team relationship. The Michigan Department of Community Health is the lead oversight agency for MiPCT, the largest Patient-Centered Medial Home (PCMH) project in the nation.

Patient-Centered Medical Home designation means that our Caregivers, led by primary care Physicians, are proactively managing our Patients’ care across health care settings – focusing on wellness, disease management and Patients’ unique personal health goals.
