Be Well at Sparrow is an easy-to-use, comprehensive and confidential online health management system that allows Sparrow caregivers to track their weight and food intake, log physical activity, measure success, get health reports, customize a wellness plan, challenge fellow caregivers and much more.

Access the online tool from anywhere

Caregivers can access the portal from any computer or a smartphone. By participating and working toward goals, users can earn points and win prizes, including iPads, iPods, Kindles, gas cards, and gift certificates.

Health assessment

Your path to wellness begins by completing a Health Assessment. This 40-question survey takes approximately 15 – 20 minutes to complete. If you do not answer all 40 questions at first, your answers will be saved and you can log-in at a later date to continue. Upon completion, you will receive a Health Report and Risk Advisor to help you understand your current state of health and create a Wellness Plan.

Food log

Our food log and meal planning capabilities enable you to easily log your caloric intake or choose from our user-driven meal plans, offering millions of possibilities for each day.

Fruit & veggie tracker

The Fruit & Vegetable Tracker allows you to quickly and easily log and track the number of fruits and vegetables you eat on a daily basis. The tracker includes six categories; three fruit categories and three vegetable categories. Each category is separated according to similarity in calories and nutritional value.

Weight log

Log your weight each day to help you track your progress and adjust your plan to meet your goals.
