CARSON CITY, MI – Sparrow Carson Physical Rehabilitation is excited to announce the return of its satellite clinic to Ithaca.
Located inside Sparrow Medical Group Ithaca at 114 N. Main Street, Patients will receive the same outstanding care available at Sparrow Carson Hospital but closer to home.
Cheryl Bazan, DPT, CKTP, will see Patients at the Ithaca location on Mondays, Wednesdays, and Thursdays from 7:30 a.m. to 4 p.m. for orthopedic post-operative and non-surgical orthopedic care, sports medicine, and common neurologic conditions. Bazan is also certified in Kinesio taping, a therapeutic taping technique to facilitate the body’s natural healing process while providing support and stability to muscles and joints without restricting the body’s range of motion.
“Patients receive the same personalized, one-on-one care as they would at Sparrow Carson Physical Rehabilitation Center. Reopening the Ithaca location allows us to provide accessible services to Patients in an expanded area,” says Monte Shearer, Sparrow Carson Hospital Physical Therapy Manager.

Sparrow Carson Physical Rehabilitation Center is open Monday, Wednesday, Thursday and Friday from 7 a.m. to 5:30 p.m. and Tuesday from 7 a.m. to 5 p.m. It is located at 216 Fifth Street in Carson City and offers the same great service as Ithaca Physical Rehabilitation, as well as aqua therapy.
