Part of Sparrow’s mission is to improve the health of the people in our communities by providing quality, compassionate care to everyone, every time. In conjunction with that mission, the Sparrow Mobile Health Clinic is dedicated to providing care to those in our communities who lack access to healthcare through traditional avenues. Under the direction of the Population Health Service Organization (PHSO), the Mobile Health Clinic is used and requested often to provide healthcare in the community and highlight different Sparrow initiatives. The mobile clinic is entirely funded from the generosity of donors, on an annual basis, and the W3 Mobile Health Endowment.

The Sparrow Mobile Health Clinic is staffed by physicians from the Sparrow Family Medicine Residency Clinic and nurses from the Population Health Department. Our team offers a variety of services to our patients including various health screenings, immunizations, physician visits, and patient education.

To request the use of the Mobile Health Clinic please fill out the form below. To allow for proper event coordination, please submit your request at least 20 days in advance. The PHSO will review submitted requests and based on the information included in the Event Request Form, will either approve or deny your request. You will be notified of the decision within ten business days after submission.
